Missing SRT

Hey guys I will make it short, I'm waiting for my SRT creds to show up since a full week here is what I've tried:

The project category is not showing in discourse therefore i can not contact to a QM
I tried to contact another category's QMs but they did not even reply!!
I sent so much support tickets from outlier dashboard with no response to it
The task is available in my dashboard but i have to log in to SRT platform with SRT creds
I heard people saying something about "Google form for missing SRT creds" if anyone have any idea how to get it?
