Friend request approval- However I never sent a request. Anyone else experiencing this?

Over the past week daily I am getting several notifications of friend request approvals to people I never sent requests to. I have changed my password and set up extra security features to prevent this further however it keeps happening. Does anyone know why this is happening or have you experienced this as well? I’m getting pretty annoyed with it.
